New Passport (DS-11 Form)

Requirements (at least one):
  • You are applying for your first U.S. passport.
  • You are 16 or older and your previous U.S. passport was issued when you were under 16.
  • Your previous U.S. passport was issued more than 15 years ago.
  • You are applying OR renewing a passport for someone under 16.

Passport Renewal (DS-82 Form)

Your most recent U.S. passport is one of the following:
  • Expiring or has expired in the last 5 years.
  • Low on blank passport pages.
  • Issued more than one year ago and you legally changed your name.
Reminder: Passport must be in your possession, not damaged, and issued when you were 16 or older.

Lost, Stolen, or Damaged Passport (DS-64 Form)

Requirements:
  • Your previous U.S. passport was lost or stolen.
  • You must complete both the DS-11 and DS-64 (Statement Regarding Lost or Stolen Passport) forms.

Corrections and Amendments on Passports (DS-5504 Form)

Used for Corrections, Name Change, within 1 year of Passport Issuance, and limited passport holders.
  • Attach your most recent U.S. passport book and/or passport card.
  • Submit recent color photograph.

Additional visa pages or miscellaneous passport services (DS-4085 Form)

Attach to this form is your valid U.S. passport book and your fee for additional visa pages.
